reformated MARK book

I've recompiled the MARK book, adding 'bookmarks' to the PDF file - so that the TOC (Table of Contents) is now 'hyperlinked', meaning you can move around the book more efficiently than previous versions (increasingly useful now that the book is >750 pages. Edition 7 - pending June, 2008 - will be ~1000 pages).

I resisted 'hyperlinking' the index, for heuristic, and technical reasons. I may revisit this decision later, but for now...

The only downside to this little exercise (besides the 2 days or so it took me to figure out how to make it all work) is that the PDF file is marginally bigger (14 Mb, vs 11-12). Now that most folks have a decent high-speed connection, this difference is negligible, perhaps, but thought I'd mention it.

This new version also has a few tweaks, and some minor additions. I'm not doing much more with edition 6 at this point beyond 'bug fixes', and am concentrating on finishing edition 7.
